Requirements
- Master’s degree in a social work or social welfare program accredited by the Council on Social Work Education
- MSW licensure by the South Carolina Board of Social Work Examiners
- Certification in case management preferred
Responsibilities
- Assess psychosocial needs and apply interventions to support the patient’s medical treatment plan
- Utilize interventions to enhance patient and family/caregiver functioning and emotional well-being
- Collaborate and coordinate care to enhance the patient experience and achieve collaborative goals
- Develop patient goals, monitor progress, address barriers to goal achievement, and evaluate outcomes of individualized care plans
- Provide training, guidance, and mentorship to staff and physicians on managing complex psychosocial patient issues
Preferred Qualifications
