Requirements
- Bilingual, English, and Spanish.
- Must reside in Miami-Dade Florida, Applicable Zip Codes 33172, 33174, 33183, 33184 or 33186
- Must possess reliable transportation and be willing and able to travel up to 75% of the time to meet members face to face in Miami Dade county FL. Mileage is reimbursed per our company expense reimbursement policy
- 1+ year of experience in behavioral health or a long-term care setting
Responsibilities
- Telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness.
- Effectively manage a caseload that includes supportive and medically complex members.
- Develop a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration.
- Determine appropriate services and supports due to member’s health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports.
Preferred Qualifications
- Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
- Effective communication skills, both verbal and written
- Managed Care experience
- Computer proficiency in Microsoft Word, Excel, and Outlook required
- Case management and discharge planning experience
