Requirements

  • BA and/or relevant experience as determined by Supervisor
  • Two or more years of direct practice experience working with seriously emotionally disturbed children, adolescents, and families
  • Experience in engaging parents/caregivers as partners and working successfully with families from diverse ethnic and cultural groups
  • Ability to work flexible hours, including occasional night/weekend work
  • Excellent person relationship, facilitation, and service management skills
  • Experience in crisis management is helpful.

Responsibilities

  • Oversee the development and implementation of Individualized Care Plans/Health Action Plans to ensure comprehensive and person-centered care.
  • Regularly communicate with other service providers to ensure continuity and integration of care across all involved parties, including primary care, mental health, substance use disorder treatment, and necessary community-based and social services.
  • Provide support to engage clients in their treatment, including medication review and reconciliation, appointment scheduling, transportation coordination, accompaniment to critical appointments, and addressing barriers to client engagement in treatment.
  • Communicate clients’ needs and preferences promptly to all members of the care team in a manner that ensures safe, appropriate, and effective person-centered care.
  • Maintain regular contact with clients and their families, guardians, caregivers, or authorized support persons to ensure consistency of care plan implementation.
  • Engage in patient interaction per eligibility requirements and standards as set by the program.
  • Participates in the development of clinical strategic and operational plans along with social workers, and medical providers and provides leadership needed to achieve medical goals.
  • Completes timely reports and coordinates with insurance companies for approval and authorization.
  • Able to perform complete enrollment evaluations.
  • Provides regular communication with the staff regarding pertinent organizational and patient issues.
  • Communicates with all members of the multidisciplinary team to improve the quality of patient care.
  • Participates in patient care conferences and makes regular entries in inpatient care logs.
  • Responsible for coordinating with those individuals and/or entities to ensure a seamless experience for the patient and non-duplication of services.
  • Offer services where the patient lives, seeks care, or finds most easily accessible and within Managed Care plan guidelines.
  • Electronic Health Record Management: Utilize EMR/EHR/Care Management Software effectively to document and track client interactions and progress.

Preferred Qualifications

  • Experience in crisis management is helpful.
