Requirements
- Active and good standing Ohio RN License
- 3+ years of clinical experience
- Must possess reliable transportation and be willing and able to travel 50-75% of the time in the following counties: Wood, Fulton, Lucas, Ottawa.
Responsibilities
- Assessing, planning, implementing, and coordinating all case management activities with members
- Evaluating the medical needs of the member to facilitate the member’s overall wellness
- Developing a proactive course of action to address issues presented to enhance short and long-term outcomes
- Conducting evaluations of member’s needs and benefit plan eligibility
- Utilizing case management processes in compliance with regulatory and company policies and procedures
- Using interviewing skills to ensure maximum member engagement and discern their health status and health needs
Preferred Qualifications
- Home Health preferred
- Computer Skills (Microsoft office such as: Word, Excel, and outlook)
