Requirements

  • 2 years of community health work
  • Demonstrated ability to successfully engage community health organizations
  • Proven ability to work with cross functional teams
  • Excellent oral and written communication skills; experience giving presentations
  • Experience using Microsoft Office suite, including PowerPoint and Excel
  • Willingness to travel regionally to events, up to 50% travel

Responsibilities

  • Outreach to ECM providers, Federally Qualified Health Centers (FQHCs), hospitals, Long Term Care (LTC) facilities and Community Health Worker (CHW) organizations to engage and provide overview of MM services
  • Create and maintain target organization contact list
  • Ensure all contacts and locations for all target organizations are up to date in internal system
  • Offer and schedule onsite (preferred) or virtual in-services with target organizations
  • Be highly visible within NYHER communities by engaging in community-based events
  • Attend and participate in county round tables and county and state webinars to represent Mom’s Meals
  • Visit Provider groups: Maternal Health practices, behavioral health, community transitions, local health clinics, FQHCs, WIC locations and others and provide overview of MM services
  • Stay up to date on referral workflows and Populations of Focus from NY Department of Health and contracted health plans
  • Stay up to date with Community Supports integration with closed loop referral systems and Mom’s Meals presence within systems
  • Stay up to date on NY Department of Health and health plan reporting requirements and work internally to ensure reporting meets requirements

Preferred Qualifications

  • Associates or Bachelor’s degree
  • Experience working with NY Medicaid
  • Experience working with food and/or health care organizations
  • Study or work in social services or nutrition
