Requirements
- Valid LVN/LPN license in the state of CA
- Current BLS/CPR certification
- Strong clinical skills with the ability to work both independently and as part of a team
- Excellent communication and interpersonal skills
- Compassionate, patient-centered approach with a commitment to quality care
Responsibilities
- Provide direct patient care under the supervision of a Registered Nurse (RN) or physician
- Administer medications and treatments as prescribed, ensuring patient safety and adherence to protocols
- Monitor and document patient progress, including vital signs, symptoms, and responses to treatment
- Collaborate with the healthcare team to develop and implement individualized care plans
- Educate patients and their families about health conditions, treatment options, and self-care techniques
- Assist with daily living activities and ensure a comfortable and supportive environment for patients
- Respond to patient concerns and changes in condition promptly and effectively
Preferred Qualifications
- Previous experience in a rehabilitation or long-term care setting preferred
