Requirements
- Effectively communicate via written, verbal, in person and virtual methods.
- Customer service skills.
- Time management skills.
- Analytical thinking skills.
- High School diploma or GED required.
- BA/BS from a four-year college/university preferred.
- Two years of direct service experience working or volunteering in a human service organization.
Responsibilities
- Provides individual, supportive coaching to improve physical health status for assigned clients, such as help in scheduling appointments with doctors, coaching on communicating more effectively with doctors and following instructions on medications or other doctor recommendations.
- Coordinates client care with team of internal/external providers. Obtains *Release of Information* for all medical providers for coordination of care.
- Provides life skills training, care management, and structured interventions to assist consumers in gaining access to resources and the skills necessary to improve their physical health status, quality of life, and self-management of symptoms. This includes areas like health hygiene, lifestyle habits, nutrition, reducing sedentary behavior, healthcare navigation.
- Provides support across the phases of care that is responsive to the changing needs of the persons served.
- Understands how the job functions within the overall Certified Community Behavioral Health Clinics (CCBHC) model.
- Perform all required job duties and responsibilities in accordance to CCBHC Standards.
