Requirements
- MSW, Graduate of an accredited school of social work, sociology, or related field, required.
- Current license in good standing as a Medical Social Worker (LSW or LCSW) in the state of practice, required.
- Ability to maintain reliable transportation, maintain a current drivers license in good standing and current auto insurance, required.
- Current Basic Life Support for Healthcare Providers (BLS-HCP) course completion card from the American Heart Association, required.
Responsibilities
- Utilize clinical expertise to perform psychosocial assessments.
- Develop and implement plans of care appropriate for the care team.
- Assess crisis situations to appropriately provide needed resources and or referrals for the patient.
- Provide family and individual treatment as indicated.
- Assess for appropriateness of level of care setting from admission through discharge.
- Identify patients requiring case management and take the lead as care manager for those requiring intervention, especially with socially complex cases.
- Collaborate with the health care team to develop a plan of care by participating in patient care rounds.
- Assure the plan of care is individualized for patients with a chronic disease to support patient/family self-management.
- Serve as a support to nursing partners in the area of social services, Medicaid applications, disability eligibility, ETOH resources and homeless resources.
Preferred Qualifications
- Certified Social Worker in Health Care (C-SWHC), preferred.
- Advanced Certified Hospice and Palliative Social Worker (ACHP-SW), preferred.
- Advanced Palliative Hospice Social Worker Certification (APHSW-C), preferred.
