Requirements
- Graduate of an accredited practical nurse or vocational nursing program.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the State.
- Demonstrates good verbal and written communication, and organization skills.
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
- Possess and maintains current BLS CPR Certification.
Responsibilities
- Provide direct patient care as per State Nurse Practice Act.
- Follow the care plan set by the clinician.
- Document care accurately and on time.
- Educate patients and families about the diagnosis and care plan.
- Coordinate home health services and report needs for other disciplines to the clinician or manager.
- Use equipment and supplies efficiently.
- Engage in personal and professional development.
- Perform other assigned duties.
- Be supervised by an RN monthly as per agency policy.
Preferred Qualifications
- One (1) year nursing experience in home care, hospice care, institutional hospital or facility care preferred.
