Requirements
- Genuine passion for primary care and reaching under-resourced communities
- Strong clinical skills focused on older adults with complex disease states
- Experience in home-based care (as RN or APN)
- Excellent communication, collaboration, and problem-solving skills
- Have an active, non-probationary state medical license, including US work authorization, if applicable
- 1+ years APN experience, preferably with a Medicare population
- Access to reliable transportation
- Flexibility to travel around the local market
- US work authorization
Responsibilities
- Conduct medical house calls for homebound patients according to defined pathways
- Optimize and monitor clinical status, identify and address gaps in care, reconciling medications and addressing adherence challenges
- Collaborate with home health, specialists, social workers, hospitals, SNFs, and specialists
- Ensure clinical excellence, compliance with regulatory guidelines, and promotion of the Oak Street Health care model
- Other duties, as assigned
Preferred Qualifications
- Bilingual (English and Spanish) preferred, but not required
- Working knowledge of Word, Excel and EMRs
