Requirements
- At least 5 years Primary Care experience
- Ability to see/treat all age ranges from Pediatric (birth) to Geriatric
- Completion of an ARC-PA approved physician assistant program (for Physician Assistant candidates)
- Current NCCPA certification or eligibility and Pennsylvania State licensure (for Physician Assistant candidates)
- Certificate of completion from an approved program for Nurse Practitioners or eligibility for national certification (for Nurse Practitioner candidates)
- Eligibility for licensure as a nurse practitioner in the state of PA
Responsibilities
- Participate in daily huddles with care team members to review coordination of care
- Assist with patient education and medication management with pharmacists
- Utilize Epic EMR system
- Manage patient appointments, including 20-minute and forty-minute appointments for elderly patients
- Work collaboratively with care managers, health managers, clinic health associates, and coding educators
