Responsibilities

  • Conduct comprehensive assessments to identify early signs of CHF exacerbation, enabling timely interventions to prevent hospitalizations.
  • Develop and implement personalized treatment plans aimed at achieving maximal doses of GDMT, optimizing heart function, and reducing symptoms.
  • Carefully titrate medications to reach target doses of GDMT, monitor for side effects, and ensure patient adherence to therapy.
  • Educate patients and their families on CHF management, emphasizing the importance of medication adherence, lifestyle changes, and recognizing early symptoms of worsening heart failure.
  • Proactively manage symptoms through home-based interventions, such as diuretics adjustments and dietary modifications, to prevent acute exacerbations.
  • Collaborate with a multidisciplinary team, including cardiologists, home health nurses, and social workers, to provide comprehensive, coordinated care in the home setting.
  • Conduct regular home visits or telehealth consultations to monitor patient progress, adjust treatment plans, and provide ongoing support.
  • Provide immediate care for acute CHF symptoms at home, reducing the need for emergency room visits and hospital admissions.
  • Ensure patients have access to necessary resources, such as home health services and support groups, to enhance their quality of life.
  • Participate in research and quality improvement projects focused on home-based CHF management and the effectiveness of GDMT in reducing hospitalizations and improving patient outcomes.

Preferred Qualifications

  • Certification in cardiology or CHF management would be ideal but not required
  • Experience in managing CHF patients, particularly in a home-based setting is ideal but not required
