Responsibilities
- Conduct comprehensive assessments to identify early signs of CHF exacerbation, enabling timely interventions to prevent hospitalizations.
- Develop and implement personalized treatment plans aimed at achieving maximal doses of GDMT, optimizing heart function, and reducing symptoms.
- Carefully titrate medications to reach target doses of GDMT, monitor for side effects, and ensure patient adherence to therapy.
- Educate patients and their families on CHF management, emphasizing the importance of medication adherence, lifestyle changes, and recognizing early symptoms of worsening heart failure.
- Proactively manage symptoms through home-based interventions, such as diuretics adjustments and dietary modifications, to prevent acute exacerbations.
- Collaborate with a multidisciplinary team, including cardiologists, home health nurses, and social workers, to provide comprehensive, coordinated care in the home setting.
- Conduct regular home visits or telehealth consultations to monitor patient progress, adjust treatment plans, and provide ongoing support.
- Provide immediate care for acute CHF symptoms at home, reducing the need for emergency room visits and hospital admissions.
- Ensure patients have access to necessary resources, such as home health services and support groups, to enhance their quality of life.
- Participate in research and quality improvement projects focused on home-based CHF management and the effectiveness of GDMT in reducing hospitalizations and improving patient outcomes.
Preferred Qualifications
- Certification in cardiology or CHF management would be ideal but not required
- Experience in managing CHF patients, particularly in a home-based setting is ideal but not required
