Requirements
- Strong work ethic and relationship building skills.
- Self-motivated with the ability to manage time appropriately.
- Strong communication skills inside of the office and outside.
- Experience with MS Office products (Outlook, Word, Excel, etc.)
- Minimum associate level education preferred. (High school diploma or equivalent is required.)
- Minimum 1-3 year of experience in a Care Team or Patient Navigation role.
- Dependable transportation to be able to travel locally between locations including patient homes, physician offices and Giving offices.
Responsibilities
- Receive the patients from Patient Development department through a warm handoff process.
- Attend the patients’ doctor visits to provide support, address concerns, and facilitate communication between patients and healthcare providers.
- Conduct home visits to assess patients’ needs, deliver resources, and provide additional assistance as required.
- Conduct an initial set up home visit for patients newly approved for home health care. Provide information on the program, as well as facilitate caregiver onboarding.
- Deliver comprehensive patient binders to patients’ homes containing plan of care and training information.
- Serve as the main point of contact for patients, ensuring they have a dedicated resource for all inquiries, concerns, and updates regarding their care.
- Make courtesy calls to patients to check on their well-being, address immediate concerns, and maintain open lines of communication between the patient and the Giving Home Health Care clinical team.
- Conduct milestone calls to track patient progress, update them timely regarding their care, approvals, caregiver status, and any other updates to their care.
- Act as a patient advocate by collaborating with healthcare providers to address patient concerns and ensure patients receive high-quality, personalized care.
- Advocate for patients’ needs and rights within the organization, promoting a patient-centered approach to healthcare delivery.
Preferred Qualifications
- Healthcare experience preferred.
