Requirements

• Associate Degree or High school degree with Technical Training, such as Medical Assistant or other healthcare training.

Responsibilities

• Coordinating activities surrounding ‘Patient Centered Care’.
• Identifying current and new patient populations through risk stratification lists.
• Supporting and assisting patients to navigate through all the steps in obtaining the necessary and appropriate care.
