Requirements
- Licensed Pharmacist
- RPh or PharmD plus five (5) years relevant experience or PharmD plus completion of ASHP – Accredited PGY1 residency training program
- Tennessee Board of Pharmacy Pharmacist License must be obtained within 90 days of employment
Responsibilities
- Work directly with Pulmonology providers and patients, helping to provide comprehensive pharmacy services to patients in the outpatient setting
- Medication reconciliation
- Optimizing pharmacotherapy
- Patient counseling
- Prior authorization review and approval
- Assisting with retail prescriptions
- Working with the patient to navigate outpatient pharmacy services
- Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications
- Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy
- Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems
- Maintains the quality, safety, and consistency of clinical Pharmacy services in assigned areas
- Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients
