Requirements

  • Graduate of or candidate for graduation from an ACPE-accredited Doctor of Pharmacy program (or one in the process of pursuing accreditation) or has a Foreign Pharmacy Graduate Examination Committee (FPGEC) certificate from the NABP
  • Eligible for licensure as a pharmacist in the commonwealth of Kentucky by the start of the residency program
  • Kentucky Board of Pharmacy intern license at the start of the residency program
  • Kentucky Board of Pharmacy pharmacist license within 90 days of the residency start date

Responsibilities

  • Supervising and performing pharmacy operations in accordance with hospital policies and procedures, ethical and professional practices, accreditation standards, State and Federal requirements and National Patient Safety Goals regarding medication management
  • Interpreting and reviewing medication orders for correct drug, dosage, route, frequency, allergies, therapeutic duplication, interactions, contraindications, and potential adverse events for all ages served
  • Ensuring drugs are available to patients at the time they are needed to avoid delays in therapy and promote positive patient outcome
  • Performing pharmaceutical calculations
  • Prioritizing workflow by patient impact
  • Supervising/performs production of compounded products (sterile and nonsterile, hazardous and nonhazardous) for first doses, scheduled deliveries and batched products
  • Providing technician oversight and assumes role of supervisor in routine daily activities
  • Overseeing dispensing and reconciliation of medications including controlled substances; management of expiration dates and drug recalls
  • Maintaining knowledge of local, state and federal regulations regarding pharmacy practice as well as knowledge of all applicable accrediting bodies medication management standards (e.g. the Joint Commission, HFAP, etc.)
  • Demonstrating supervisory skills necessary to plan, organize and direct activities in the pharmacy in a cost effective manner
  • Effectively utilizing pharmaceutical references
  • Serving as a drug information expert and resource regarding medication management
  • Having a working knowledge of pharmacy operations including: pharmacy operating system, use of patient record, purchasing, inventory and formulary management, automation, packaging, USP 797, USP 795, USP 800, and pharmaceutical waste
  • Initiating, documenting, and participating in department activities that promote positive patient outcomes
  • Assisting in the orientation and training of technicians and students
  • Serving as an active member of the inter-professional team on patient care units
  • Assisting in data collection as assigned
  • Promoting formulary therapies to the medical staff
  • Providing medication counseling to patients
  • Performing medication reconciliation for patients
  • Working with inter-professional patient care teams to help patients with transitions to care
  • Acting as preceptor for students as applicable
  • Communicating with Residency Program Director when needed and in appropriate time frame
  • Developing a project to be presented at a pharmacy conference
  • Responsible for documented completion of residency program completion requirements as outlined in the training agreement
