Requirements
- Requires in depth understanding of a comprehensive field of knowledge normally associated with the attainment of a Bachelor’s degree in human services or related field. Bachelor’s degree required.
- Ability to acquire certifications in SAMA, CPS/ First Aid, Medications and other clinical training as required.
- Requires ability to drive assigned vehicle(s) or personal vehicle, with appropriate state license, following all laws applicable; must provide proof of liability insurance and must be eligible to be insured under Buckner’s insurance policy. Must be age 21 or older to drive on behalf of Buckner.
- Requires ability to use up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people.
- Requires ability to walk, stand and sit, sometimes for prolonged periods of time.
Responsibilities
- Participate as a member of the Post Permanency team to work with the foster care and adoption team to meet the physical, emotional and social and spiritual needs of foster or adopted children after they have been discharged from a Buckner home.
- Work alongside the Post Permanency Director and the Post Permanency Specialist to serve children after they have been discharged from a Buckner home.
- Collaboration with the child’s core Service Planning team and include: the child, the child’s parents, the child’s CPS or SSCC caseworker, attorney ad-litem, guardian ad-litem and/ or CASA volunteer, STAR Health Service Coordinator, relatives, subsequent caregivers and other stakeholders.
- Assistance with school enrollment (if applicable per the child’s age) prior to discharge and assistance with development and maintenance of the Educational Portfolio.
- Assist with referrals for normative, age-appropriate, positive peer relationships and activities within the child’s community at the subsequent placement.
- Advocate for families and clients within social service systems, to include educational and medical.
- Assist with the transition of clients to other medical and mental health providers, as needed. This includes collaboration with child’s Service team, Buckner Foster Care and Adoption and the Post Permanency Specialist to ensure that there is no lapse in therapy or medication, if applicable.
- Use a collaborative and collegial approach to engage and motivate families.
Preferred Qualifications
- Two years prior related work experience in the field of adoption or foster care preferred.
