Requirements

  • High School Diploma or equivalent required.
  • Bachelor’s Degree in Social Services or other health-related field preferred.
  • Minimum of 4 years of experience in a human services field is required (e.g., mental health, substance use disorder services, homeless services, etc.), and previous experience providing HIV prevention or case management services is required.
  • Minimum of 4 years of professional experience in Motivational Interviewing and/or demonstrated capacity to deliver client-centered harm reduction counseling is highly desirable.
  • Previous experience delivering case management services to low-income, at-risk Hispanic populations preferred.
  • Existing certification as an HIV Test Counselor, Los Angeles Division of HIV and STD Programs, is strongly preferred; or must be willing to complete HIV Test Counselor training within 3 months of employment with AltaMed.
  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Responsibilities

  • Providing quality and comprehensive HIV/STI Health Education, HIV counseling and testing, and facilitating HIV Prevention and PrEP/PEP Navigation Services to clients at high risk for infection or transmission of HIV.
  • Referring clients for ACA enrollment.
  • Navigating clients onto PrEP/PEP services.
  • Scheduling medical appointments for PrEP/PEP.
  • Assisting with Medication Assistance Programs.
  • Conducting program recruitment for PrEP, client intake and assessment, facilitation/mentoring, HIV counseling and testing, as well as coordination of referral services into other programs as appropriate and necessary.
  • Monitoring, evaluating, and reporting on progress toward grant deliverables.
  • Representing AltaMed by serving on internal and external workgroups, collaborative teams, and coalitions.

Preferred Qualifications

  • Bachelor’s Degree in Social Services or other health-related field preferred.
