Requirements
- Bachelor’s Degree
- Minimum of 5 years of experience
- Minimum of 2 years supervisory experience
- Strong written and oral communication skills
- Experience in program development, implementation and evaluation
Responsibilities
- Oversee care coordination programming and Health Home program activities
- Lead day-to-day operations and monitor program requirements (billing, documentation, data reports)
- Guide implementation of the strategic vision of the program
- Supervise care coordinators by providing coaching, guidance, and feedback
- Collaborate with team members and participate in Care Coordination Steering committee meetings
- Track program outcomes and report back to various groups, including funders and senior leadership
Preferred Qualifications
- Bilingual English/Spanish
- Master’s in social work/related clinical field or Master’s in Public Health
- Experience in program management, development, implementation and evaluation
- Interest in Quality Improvement strategies/planning
- Familiarity with care coordination and program management including ensuring deliverables are met and reporting out to key stakeholders
- Comfort with data reports and tasking team members to achieve high quality outcomes for patients and meet programmatic requirements
