Requirements
- Bachelor’s Degree in Nursing
- 3 years of relevant nursing experience
- Registered Nurse (RN) – State of Michigan Upon Hire
- Basic Life Support (BLS) – AHA American Heart Association Upon Hire or Basic Life Support (BLS) – ARC American Red Cross Upon Hire
Responsibilities
- Assess the patient’s individual needs and develop a plan of care to meet those needs
- Provide education and support to patients, families, staff and caregivers
- Improve patient care and flow, eliminate barriers, and improve the overall patient experience
- Determine the appropriate appointment scheduling by assessing patient pain and severity of injury
- Coordinate the care of patients in assigned service line
- Ask appropriate clinical questions and elicit patient information to determine appropriate actions
- Access multiple network clinical information systems to gather and verify patient demographic and clinical information
- Collaborate with all care providers to assist in the provision of quality, cost effective patient care
- Provide individualized support, education and information to make care seamless, continuous and comprehensive
- Identify and document issues related to transitions of care and round on patients as appropriate
- Manage continuum of care from client facilities, giving feedback to and from Beaumont Health
- Serve as a liaison between the patient and their family and the various hospitals and physician office professionals
- Maintain a full understanding of the services provided by Beaumont Health hospitals, ordering physicians, referral centers and other organizations in the community
- Responsible for patients within the inpatient and outpatient setting
