Requirements

  • Current RN license in the state of employment
  • Minimum of Three (3) years of experience as an RN in an acute care setting
  • Valid driver’s license and auto insurance in your name as a driver
  • Capable of all physical demands

Responsibilities

  • Ensure Start of Care compliance by our clinical team and focus on Outcome Monitors as assigned
  • Participate in all liaison/administrative on-call rotation, and patient staffing as needed to manage after hour’s team, staffing needs and patient related issues
  • Process complaint workflow reporting staffing trends, neglect, abuse or theft to Administrator for resolution & follow up
  • Complete patient visits as needed and assist with any patient issues
  • Participate in managing cost of good working with monitoring supply costs, overtime, staffing, bonuses and visits per episode
  • Select and onboard staff
  • Collaborate with intake, providing daily direction, managing referral volume based on staffing availability
  • Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families
  • Provide oversight for liaison and clinical staff and ensure timeliness of completion of work/charting as well as identify education needs for the team
  • Manage On call, PTO, call offs & daily/weekly productivity review to ensure proper staffing coverage to meet the needs of the business in the field and internally
  • Ensure quality and safe delivery of home health services by our clinical team
  • Review clinical documentation and maintain current knowledge of all Medicare/ Federal/ Insurance / Company and Hospital guidelines, policies and protocol
