Responsibilities
- Conduct adult wellness visits
- Manage transitions of care
- Assist providers with patient messages
- Perform office procedures such as IV and wound care teaching
- Educate patients and families on chronic disease management
- Coordinate disease management programs
- Maintain required documentation in EPIC
- Collect and enter data and patient information according to department standards
- Facilitate communication among patients, providers, and healthcare teams
- Implement clinical interventions based on risk stratification and evidence-based clinical guidelines
