Requirements
- Basic Life Support (BLS) Certification through the American Heart Association (AHA)
- Two years of RN experience
- One year of acute care hospital experience
- Disease-related experience
Responsibilities
- Oversee and manage all transplant patient activities at Weston Hospital
- Assist fellow caregivers in creating care plans
- Review medical information and assemble test results
- Coordinate transplants and scheduling with administrative coordinators
- Coordinate consultation with other disciplines
- Assist in obtaining medications and supplies
- Contact referring physicians for follow-up
- Evaluate patients for transplant candidacy
- Assemble test results and arrange further testing
- Serve as contact person for pre-transplant evaluation
- Prepare and present cases to the Selection Committee
- Place patients on the national waiting list for transplantation
- Coordinate ongoing care of inpatients and outpatients waiting for transplant
- Triage patients during the transplant process
- Provide immediate assessment and delivery of care
- Collaborate with Office Coordinators and Financial Counselors
- Coordinate informational packets for patients
- Review information with patients and answer questions
- Coordinate and provide educational sessions for patients and families
- Collaborate with the Organ Procurement Organization
- Facilitate procedures
- Provide in-services for nursing caregivers
Preferred Qualifications
- Bachelor’s of Science in Nursing (BSN)
