Requirements

  • A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3).
  • Minimum of one (1) years’ experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
  • Demonstrates good verbal and written communication, and organization skills.
  • Current licensure according to Medicare guidelines to practice in state of operation.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Responsibilities

  • Guides and counsels individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for them.
  • Assists clients in obtaining government funds, education, or treatment.
  • Secures information, such as medical, psychological, and social factors contributing to client’s situation, and evaluates these and client’s capacities. Asks clients to explain problems, such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine nature and degree of problem.
  • Helps patient and family through individual or group conferences regarding plans for meeting needs, and aids client to mobilize inner capacities and environmental resources to improve social functioning.
  • Helps client to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and client’s part in creating them.
  • Develops service plan designed to restore patient to optimum social and health adjustment within patient’s capacity.
  • Refers clients to community resources and other organizations, and liaises with other social service agencies. Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability.
  • Prepares patient histories, service plans, and reports. Reviews service plan and performs follow-up to determine quantity and quality of service provided client and status of client’s case.
  • Maintains client records in compliance with organizational goals and objectives, as well as external regulatory guidelines.
  • Other duties as assigned.

Preferred Qualifications

  • Hospice experience preferred.
