Requirements
- MD or DO degree from a fully accredited medical or osteopathic school of medicine
- Sports Medicine fellowship trained
- Board certification or eligibility in Primary Care Sports Medicine and Board Certification in Family Medicine
- Active, unobstructed Utah medical license, or the ability to obtain one
- Ability to successfully complete Intermountain Health’s credentialing process

Responsibilities
- Establish a practice and presence in Tremonton as a strong member of the rural community
- See patients of all ages for fracture care, sports-related concussions, and procedures, including ultrasound-guided injections and orthopedic biologics (PRP)
- Cover your own patient-panel call
- Provide university sports coverage in Logan, Utah
- Provide high school sanctioned sports coverage in Tremonton, Utah
