Requirements

  • Active Substance Use Disorder Professional (SUDP) credential in good standing with the Washington State Department of Health.
  • Must pass required background checks.

Responsibilities

  • Provide one-on-one counseling and case management services, tailoring your approach to meet each member’s unique needs.
  • Help members craft personalized recovery plans, setting meaningful goals and actionable steps.
  • Facilitate group counseling sessions, creating a supportive space for peer engagement and shared progress.
  • Connect members to critical resources like housing, education, and employment, guiding them through systems to regain stability.
  • Be a strong advocate for members, ensuring they access the services they need to achieve their goals.
  • Support members in developing essential skills such as self-advocacy and building supportive relationships.
  • Act as a liaison with external organizations, streamlining access to critical services.
  • Adapt your counseling practices to the unique challenges and opportunities of mobile healthcare.
  • Collaborate with your mobile clinic team to ensure smooth operations and exceptional care delivery.
  • Create a welcoming, functional environment within the mobile clinic, making every member feel supported.
  • Maintain accurate, timely documentation of all services provided, meeting Medicaid and employer standards.
  • Ensure member records are up to date and accurate in the EMR system.
  • Participate in team meetings, contributing insights and ideas to improve care and operations.

Preferred Qualifications

  • Ability to lead and facilitate engaging group counseling sessions.
  • Strong knowledge of substance use disorder treatment models and best practices.
  • Proficiency in EMR systems and basic computer applications.
  • Excellent organizational and time management skills.
