Requirements
- High School diploma or GED
- Typically requires a minimum of 5 years experience in social services including experience in a supervisory role

Responsibilities
- Monitors TOWs in order to help the team meet their individual and team goals
- Creates a healthy and motivating work environment and atmosphere
- Creates workflows/procedures with external and internal stakeholders
- Oversees day to day operation and performance of TOWs
- Provides patient outreach and resource navigation
- Community outreach and Advocacy
