Requirements

  • Licensed Vocational Nurse (LVN) in good standing with the California State Board of Nursing
  • Experience in wound care or treatment nursing in a skilled nursing or similar setting is highly preferred
  • Ability to multitask and handle a fast-paced work environment
  • Strong knowledge of wound care protocols, infection control practices, and skin integrity management
  • Proficiency in electronic medical records (EMR) systems
  • Excellent communication skills, both verbal and written, to effectively interact with residents, families, and healthcare team members

Responsibilities

  • Wound Care Management: Assess, plan, and implement treatment for residents with wounds, pressure ulcers, and other skin conditions.
  • Monitor Skin Integrity: Regularly assess residents for changes in skin condition and implement preventative care strategies to avoid skin breakdown.
  • Documentation: Maintain accurate records of treatment plans, resident progress, and changes in condition using electronic medical records (EMR).
  • Collaborate with Care Team: Work closely with physicians, Registered Nurses (RNs), CNAs, and other healthcare team members to develop individualized care plans.
  • Resident & Family Education: Provide education to residents and their families on wound prevention, treatment, and skin care.
  • Compliance: Ensure all care is delivered in compliance with federal, state, and local regulations, including those specific to skilled nursing facilities.
  • Medication Administration: Administer medications related to treatment as prescribed, following proper protocols.
  • Infection Control: Adhere to infection control procedures and assist in the prevention of the spread of infections within the facility.
  • Emergency Response: Respond to and report any changes in residents’ conditions, participating in emergency care when necessary.
